Florida Heiken Vision Program

The Florida Heiken Children’s Vision Program provides vision examinations and eyeglasses, when prescribed, to students in need of comprehensive vision services at no cost to the student. Eligible students for the program must meet the criteria by having no insurance or vision plan, be enrolled in the free and reduced lunch program, and have failed the vision screening twice.

Schools are to distribute the Heiken parent letter and consent form to students in grades K, 1, 3, and 6 at the beginning of the school year. Parent letters and consent forms are available in different languages and are available on the Coordinated Student Health Services website at www.browardschools.com under the School Health Screenings tab.

Students who meet criteria as noted above and are eligible for the free vision services will be provided with a parent letter and consent form for the parent/guardian to complete, sign and return. Students who are determined to be eligible for services will be provided with a voucher by the Heiken Vision Program to one of three satellite locations available in the community.

Students who were referred to the program and found to have insurance will not be eligible for referral to an optometrist through the Heiken Children’s Vision Program, and the school principal will be notified. Schools are encouraged to consider arranging for the Heiken Vision Program Services to provide complete on-site eye exams with dilation at their school. A minimum of 10-12 eligible students with signed parental consent is required for on-site eye examination and dilatation to be provided at school. The provision of on-site school vision examinations is a convenient way for the parents of eligible students to obtain free vision services for their children without having to miss time from work.

Contact the Coordinated Student Health Services Department at 754-321-1575 to assist you with coordinating on-site eye exams with dilation with Heiken Vision Program Services. The school will need to ensure a designated room for eye examinations is provided and a school staff member is assigned to coordinate the group of students to be examined.
